Barack Obama, JD: United States Health Care Reform. Progress To Date And Next Steps

Importance The Affordable Care Act is the most important health care legislation enacted in the United States since the creation of Medicare and Medicaid in 1965. The law implemented comprehensive reforms designed to improve the accessibility, affordability, and quality of health care. Objectives To review the factors influencing the decision to pursue health reform, summarize evidence on the effects of the law to date, recommend actions that could improve the health care system, and identify general lessons for public policy from the Affordable Care Act. Evidence Analysis of publicly available data, data obtained from government agencies, and published research findings. The period examined extends from 1963 to early 2016. Findings The Affordable Care Act has made significant progress toward solving long-standing challenges facing the US health care system related to access, affordability, and quality of care. Since the Affordable Care Act became law, the uninsured rate has declined by 43%, from 16.0% in 2010 to 9.1% in 2015, primarily because of the law’s reforms. Research has documented accompanying improvements in access to care (for example, an estimated reduction in the share of nonelderly adults unable to afford care of 5.5 percentage points), financial security (for example, an estimated reduction in debts sent to collection of $600-$1000 per person gaining Medicaid coverage),

and health (for example, an estimated reduction in the share of nonelderly adults reporting fair or poor health of 3.4 percentage points). The law has also begun the process of transforming health care payment systems, with an estimated 30% of traditional Medicare payments now flowing through alternative payment models like bundled payments or accountable care organizations. These and related reforms have contributed to a sustained period of slow growth in per-enrollee health care spending and improvements in health care quality. Despite this progress, major opportunities to improve the health care system remain. Conclusions and Relevance Policy makers should build on progress made by the Affordable Care Act by continuing to implement the Health Insurance Marketplaces and delivery system reform, increasing federal financial assistance for Marketplace enrollees, introducing a public plan option in areas lacking individual market competition, and taking actions to reduce prescription drug costs. Although partisanship and special interest opposition remain, experience with the Affordable Care Act demonstrates that positive change is achievable on some of the nation’s most complex challenges.

    On Bernie being booed for telling his followers to support Hillary (according to news stories) I find that very interesting… I have always believed that most of his supporters are rightwingers/ repubs. When I was at the party I was kicked out of, when someone mentioned Bernie, I stated that he was a ‘nut’… I was basically booed for saying such a thing. I suspect that most of the people in that room were also republicans. Anyhoo… heres one test Bernie could try: Tell his supporters he’s going to support Trump- bet they’d cheer him.

    Sanders did not confront the booing delegates directly. He did not use this opportunity to address their anger and disappointment. He looked a bit surprised by the intensity of the Clinton opposition. He moved ahead with his prepared speech. After he was done—and the crowd had chanted, “Thank you, Bernie!”—Mother Jones asked Sanders three times what he thought about his delegates fiercely booing Clinton. He did not respond and quickly left the ballroom.

    So how much will Sanders do to persuade his delegates to follow his advice? In this appearance, he didn’t go beyond his prepared remarks. And Aisha Dew, a whip for the North Carolina Sanders delegation, said she has not heard anything from the Sanders campaign regarding the actions delegates should take. Earlier in the day, at a Florida delegation breakfast, Sanders offered no instructions on how delegates might conduct themselves on the floor of the convention this week.

    After Sanders departed the convention center, his campaign manager Jeff Weaver remained as delegates milled about and wondered what would be next for them at the convention. What about the booing of Clinton? he was asked. “Oh,” he said, with a tone of nonchalance, “people will come around.”

    three things:

    – I wrote here yesterday that the BS revolutionaries would NOT be satisfied by asking DWS to resign and that they would just pocket that and move on with new demands without batting an eye. they are doing so now and they feel emboldened by the resignation of DWS;

    it is not hard to find evidence that extremists just move the goal posts once you create the space for them to feel victorious;

    – why do we believe that the content of these stolen emails has not been doctored by the authors who clearly have an agenda;

    – the media will milk the email situation and the antics of the BS revolutionaries for ratings and a story line of chaos;

    My daughter-in-law’s brother asked to ‘friend’ me on FB yesterday. Seeing lots of interesting stuff from this RW Texas attorney, mostly pro-Johnson. Occurs to me that the Rs who cannot make themselves vote for HRC and also are smart enough to recognize DT as a monster might just switch to this SW Libertarian in quiet droves.

    As for the Bernt-outs, I’m guessing a lot of the boo-ers are Independents who were never going to vote for the Dem candidate anyway– not ready to declare a conspiracy, but it does seem they’re working for the other side, now that their candidate has asked them to vote Dem.

    Nothing is certain, except that we have work to do.

    If the Bernie crazies plan a floor fight and intend to enter Bernie’s name in nomination for president or vice president, he has one simple solution for dealing with that. He can do what HRC did in 2008 and ask that his name NOT be put in nomination and that Hillary Clinton be nominated by acclamation. Democratic Senate leaders like Reid and Schumer should be on the phone with him right now laying out his “options”–a momentary ego boost or a blighted and short-lived Senate career. If that doesn’t work, we may see some behind the scenes action by the President. PBO has handled far greater international and domestic challenges than Berniebots and I have no doubt he, in concert with HRC, will take firm and resolute control of this convention. One thing that PBO will not tolerate is disrespect of FLOTUS! I would not want to be Bernie Sanders of his bots boo her speech.

    This is a damn shame! It is terrible what this group is doing to Hilary.
    Nina Turner and Ben Jealous, not to mention Cornel West are great disappontments. To speak racially, do they not realize that the white people booing Clinton/Kaine are going to be fine no matter who wins? It will be the munirities, poor whites, Hispanics, immigrants and women who are going to catch hell if Hilary loses.
