My goddaughter is a surgeon, with a year left on her residency. We’ve had very interesting discussions of the state of American health care. She says that most of the developed nations (and some Third World) pay for all medical training. It varies from country to country of course, but there is always a component of a period of time you ‘serve’ where you’re needed.
Her point: if young American doctors weren’t saddled with crippling debt, many more talented young people would choose to become doctors, particularly women. And if the crippling costs of a medical education were effectively eliminated, doctors would choose what the LOVE to do, rather than what they have to do to pay off their loans and make their ‘investment’ worthwhile. Read: less dermatologists and more doctors in rural areas and better general practitioners.
Her assertions were confirmed. I remember reading about a study about 5 years ago about the choices women make in the medical field. It was not about apples-to-apples: a female cardiac surgeon compared to a male cardiac surgeon, for example. The study said yes – women doctors make less than male doctors. But it was because, if given the opportunity to make a choice, women chose to work in low income clinics, charity medical work like Doctors Without Borders, family medicine. They were willing to serve their community through medicine, rather than just make money. That is, if they could afford to make those choices.
There used to be a program where the government would reduce your debt for every year you spent serving in an underserved community. It’s one way they got doctors onto Indian reservations. I don’t know how many took advantage of it, though.
The scholarship program was renewed in the ACA bill. For four years service in an undeserved community you get tuition and a stipend for four years of medical school. I had a National Health Service scholarship and served 4 years at a Bureau of Prisons facility. Indian Health Service also counts.
Very thoughtful commentary. I do agree that medicine should be available for those with the aptitude and desire and the training should not be crippling. Over the years I have seen committed MD’s in both genders, but I am not in the US system so I don’t know what motivates. I believe many, male and female start out with the desire to serve but overwhelming debt does drive a different agenda. Congrats for your daughter. You must be filled with pride for her achievements.

He made a verbal agreement with Tesla for some important invention (I admit I forget which) to pay him $50,000 when it was finished. This does not detract from his obvious genius and one of the great quotes of all time: “Invention is 10% iinspiration and 90% perspiration”.
Now Al gore is pressing the President on carbon rules calls Keystone an atrocity . Yesterday, it was Sanders who said the same thing.
Well Al, you’ve gone all in on criticizing the Prrsident trying to undermine his authority with all these statements everyday.
But, if Hillary wins, if…, will you do the same to Her?
And God forbid, if Christie wins, will you slam him every day, as well?

2. They have a lot of data but still can’t think for themselves;
3. They are supposed to help you solve problems, but half the time they ARE the problem; and
